Extention of access to ARV therapy in Russia

Issues: Dramatic increase of the numbers of HIV-infected patients began in 1996, when the virus started to spread among the intravenous drug users. The number of the patients living with HIV by the end of 1995 was 1100, and by the end of 2007 - 415301. Most of the patients were infected in 1991-2001.
Description: Dynamics of the patients requiring HAART administration was calculated using the model developed in Russian Federal AIDS Center. This model was created based on the analysis of annual numbers of new cases, probability of death of HIV-infected patients, evidence of indications for HAART administration.

Lessons learned: By the end of 2008 the number of HIV-infected patients requiring HAART will reach 116000. The estimated number of patients who will be receiving HAART is 50000. This number is limited not because of the financial restrictions, but mainly because of insufficiency of trained personnel, local public health resources and organizational difficulties.
Next steps: Centralized support of the government including the centralized purchase of the medicines and tests systems for the management of HIV-infection has significantly improved the access to HAART. The measures for the further improvement are to be considered.
